Mueller Youth Scholarship Application

Deadline for Applications-April 14, 2021

Please note: Your application will not submit if you do not include the required fields. 
Upon submission you will receive a confirmation email. Please keep this confirmation to verify your submission. If you do not receive an email confirmation, your application submission failed. It is your responsibility to resubmit or contact the Foundation before the due date at 937-208-2700, or you will be disqualified.  The essay questions change each year. If you submit the same essay you submitted last year, you will be disqualified.
We require an acceptance letter for new high school graduates, or, for renewing applicants, a copy of your summer/fall 2021 class schedule. If you do not have these documents now because you are still working on your plans, you are required to return to our website and submit the appropriate document on the Mueller Youth Scholarship Final Document Upload page before 11:59 pm on May 21, 2021. Again, please look for the confirmation email. If you do not receive one, please contact the MVH Foundation office. This information is a requirement of the scholarship. Missing documents will disqualify your application. 

The required information must be completed online (no exceptions). Please submit all of the following materials together by 11:59pm, April 14, 2021:

  • A copy of your most recent high school/college report card.
  • First Time Applicants: College/University/Trade School acceptance letter. (Due 11:59pm, May 21, 2021)
    Renewal Applicants: Class schedule for fall 2021. (Due 11:59pm, May 21, 2021)
  • A 300-word essay: Research at least one local philanthropist who has had a meaningful impact in the medical field or cultural life of the greater Dayton area.  Discuss how you have benefited personally or been inspired by that donor’s gifts to the area.  Please include your thoughts on the importance of philanthropy and giving back to the community.
  • A separate 300-word essay: Why do you feel you should be considered for this scholarship?
