Youth Scholarship Application
This scholarship has been established by the Mueller Society Employee Council and is funded by Mueller Society donors. This program will offer a limited number of grants to employees’ dependent children pursuing training or education beyond high school.
Selection Criteria
Mueller Society Scholarships will be awarded in a competitive selection process. All applicants must meet the following criteria to be considered:
- Applicant’s parent/guardian must be an employee of Miami Valley Hospital and/or other subsidiaries. (Employees must have six months employment with MVH or its subsidiaries for dependents to apply. Contract employees’ children are not eligible.)
- Applicant must be an employee’s dependent.
- Applicant must be a high school graduate or have a G.E.D.
- Applicant should be active in one or more community service activities, such as volunteering for a non-profit organization, church group, charity walks, etc.
- Applicant must meet admission criteria and be accepted into a study program.
- Applicant must be pursuing an undergraduate level study or a trade certification.
- Application must include all items listed on the Application Cover Sheet at the time of submission.
Selection and Review Committee
A committee appointed by the Mueller Society co-chairs will review all applications and essays. Names and other identifying information will be removed prior to the selection process. All decisions will be final. The administration of the program will be coordinated by the Foundation office.
Number of Recipients
The actual number of recipients will be based upon the dollar amount assigned to the Mueller Youth Scholarship Fund by Mueller Society donors in the annual membership drive. (The larger the fund, the greater the number of scholarships to be awarded.)
Length of Participation
Participation will be approved for one year. The applicant may re-apply in subsequent years by completing a new application and submitting the required information. No more than a total of four scholarships will be awarded to one applicant. The scholarships are for undergraduate level study only.
Essay Questions Helpful Hints
When answering the required essay questions, do not include your name or refer to your parent or guardian’s job in the essay content. Please be sure to answer the question fully without quoting directly from Google, a similar information service or a dictionary definition. The judges are interested in your original thoughts on charitable giving and community service. Good Luck!

Award presentations will take place at the Mueller Society Picnic, July 26, 2012. Application deadline (no exceptions) is Friday, June 8, 2012, 4PM.
Application Cover Sheet
The required information must be completed and returned to the Miami Valley Hospital Foundation office by
4:00 PM on Friday, June 8, 2012(no exceptions). Please submit all of the following materials together:
- A copy of your most recent high school/college report card.
- One current letter of recommendation from this year.
- A typed, 300-word essay that describes your career goals, and how you intend to achieve those goals.
- In a typed one-page essay, plesse discuss your thoughts on how to invlove young people in the importance of charitable giving. While young people may not have much money to donate, describe other ways you, your friends or family have participated in volunteer or other yourth groups, church activities or charitable projects and the outcome of those projects. Please include your thoughts on the importance of philanthropy and giving back to the community.
- A typed description of current or past volunteer activities that you have contributed to or participated in, such as community service projects, events, fundraising walks or campaigns that have benefited others. Please indicate activities you have participated in during the past 12 months.
