Valley Classic Golfer Registration
Complete all information for each player, listing contact person first. Registrations cannot be processed without complete home address, GHIN#, home course, tee time, lunch/dinner reservations, shirt size and payment. (If no GHIN#, please submit your average score or handicap.) Please submit no later than July 22, 2010.
Registration: $375 per golfer, $1,500 per foursome
Mulligan ticket: $10 each and 1 per player
